Stay away from this place. It's very suspicious that they asked us for OUR social security number, place of work etc. To top it off, they offered to serve us ONLY if we gave them cash--basically asking us to give them money under the table. When we asked what they needed the number for, they just said it was their policy and repeated that we would have to pay in cash if we didn't want to fill out the form. After this, they offered to let us talk to the manager, who turned out to be the vet's daughter, who took us into a room and told us brashly that we would have to fill out the form or they would not see our dog. We asked what they needed the number for again and she told us we were playing games and that she would have to escort us out, very angrily. When we asked for the form back, since it had our name and address on it, they REFUSED, and told us to leave, repeatedly saying we were paying games. They then forced us to leave with our sick pet! HORRIBLE.
